Change of Address Form

"All About" the Change of Address Form

This form is for use by retirees or beneficiaries who are receiving a monthly pension from the National Pension Fund. If you direct deposit your monthly pension you may still use this form to report a change in your home mailing address, (i.e., the address where we will send your vouchers and any other mailings.)

Procedures

Provide all information requested including the effective date of change. Print your completed form and sign and date on the appropriate line.

DO NOT E-MAIL THIS FORM. This office must receive an original signature on the completed form in order to implement any changes. Changes may take 4-6 weeks to be effective.

Note: Completion of this form will not change your current direct deposit unless you indicate to cancel direct deposit.
