"All About" the Reactivate Pension Form

A Participant who has previously submitted an application with the Fund can use this packet of forms. You do not need to complete another application. Simply complete the reactivation form and send it to the Sheet Metal Workers' National Pension Fund. Please note that your pension can be effective the first of the month following receipt of this form. However, we recommend that you submit a pension request three to six months in advance of your intended retirement date.

Procedures

In order to promptly respond to any request for either Early Retirement or Normal Retirement, you must provide complete answers to all requested information on page(s) 1 and 2. If you also wish to be considered for an Industry Related Disability Pension, the Fund must verify through medical evidence your inability to work in the Sheet Metal Industry. In order to be considered for this type of pension, in addition to completing page(s) 1 and 2, you must also complete page(s) 3 and 4.
